    either it is not tightened or you did not apply teflon paste to the threads or if is a cartridge style check the rubber gasket in the head that the cartridge seats against

    It is not properly seated.
    remove and reinstall. if you did not use teflon tape do so when reinstalling. Any hardware store or store with a decent hardware section will have this rather cheap and well worth it not only in preventing leaks but making eventual replacement much easier.

    It is not properly seated or you cracked the housing when you installed it. Take it apart, clean it up and try again. If you cross-threaded the plastic, you need a new one.
